Referrals
Our practice requires a minimum of 48 hours notice if your child needs a referral. Although we pride ourselves on our quick response time for many of our patients, sometimes because of the high volume, it is difficult to complete everything properly in less than 24 hours. Also, it is nearly impossible for us to do a referral when you are already at the specialist for the appointment. It takes time to do a Referral properly and we cannot accept last minute requests--especially when you arrive at your specialist without your referral. So please give us at least 48 hours to complete your Referral.
Medical, School & Sports Forms
Accurately completing the necessary information for various forms for your child can be an overwhelming and time consuming tasks for our doctos and nurses. Our practice requires a minimum of seven (7) business days and a $10.00 fee to complete forms that are NOT brought in at the time of your child's annual Well Visit/Physical Exam.
In the event that you need to have a form completed within 48 Hours and was not brought in at the time of your child's Well Visit, there is a $20.00 fee for this expedited service. We cannot guarantee that any forms will be completed within 24 Hours, however we can guarantee a 48 Hour turn around time.
Chart Copy Requests
Please know that our practice follows the guidelines set by HIPPA and the New Jersey State Board of Medical Examiners regarding requests for copying patient records.
Under HIPPA guidelines, in order to receive PHI (Protected Health Information ie: Your Child's Patient Records) a Records Release Authorization must be signed by the same individual as authorized by HIPPA documentation. Also, under the guidelines set by the New Jersey State Board of Medical Examiners, our practice charges $1.00 per page. Fees need to be be collected at the front desk prior to the release of the records.
Unless, specified or special request, we copy the immunization records, problem list, the last well visit and progress sheet. If you require any additonal records the fee for all records must be paid in advance of copying of additonal records.
